REFUND REQUEST FORM

Refund Policy
Lethbridge Minor Hockey will refund the player registration fee on a pro-rated basis based on date of request.

After January 10th NO REFUNDS WILL BE APPROVED for Minor Hockey registration.

All refunds will be charged an administration fee of 10% to cover credit card or eCommerce expenses and processing fees.

Player guardian must complete the Refund Request Form, and the appropriate refund shall be calculated and forwarded.

NON Refundable Fees
-Minor Hockey registration deposit amount is NON Refundable
-Tryout Fees/ID Skates are NON Refundable

ALL JERSEYS AND EQUIPMENT MUST BE RETURNED TO RECEIVE A REFUND, IF APPLICABLE
